ETT Depth Calculator

Neonatal endotracheal tube depth & size

Educational decision support only. This is not a medical device or substitute for clinical judgment. Always confirm with your unit's protocol and applicable guidelines before clinical action.

Calculate ETT Depth (NRP rule)

Depth at upper gum
7.3 cm
Tube size
3.0 mm

NRP depth rule (8th edition)

Depth (cm at upper gum) = 6 + weight (kg)

NRP 8th edition measures insertion depth from the upper gum (alveolar ridge) rather than the lip. Always confirm placement with auscultation, CO2 detection, and chest X-ray (tip 1–2 cm above the carina, around T1–T2).

Worked example

A 1.25 kg infant: 6 + 1.25 = 7.25 cm at the upper gum.

ETT size by gestational age

  • <28 weeks — 2.5 mm
  • 28–33 weeks — 3.0 mm
  • 34–37 weeks — 3.5 mm
  • ≥38 weeks — 3.5–4.0 mm
